  Security Breach Notification

Dear Alvarado Medical Plaza Patrons,

   If you recently received a letter from Relay Health regarding a Security Breach we want you to know that we have been in contact and working with Relay Health to make sure your personal information HAS STAYED private and protected.

Relay Health's letter has stated "It is a standard practice for independent chain pharmacies [Alvarado is an independent chain, through United Drugs] to use a claims processing company such as NDCHealth Corporation (known as RelayHealth) to process and deliver prescription information from their pharmacy to your private health insurance company or government health program such as Medi-Cal."

The letter continues with "Recently, NDCHealth learned that we inadvertently sent an electronic data file, which included your prescription claims information, to another pharmacy. The electronic data file involved patient prescriptions that were dispensed at Alvarado Medical Plaza Pharmacy .... between the dates of February 2008 and December 2008"

   IF YOU are an Alvarado Medical Plaza Pharmacy Patient AND have received this letter from NDCHealth (RelayHealth) we want to assure you that we have done everything possible to make sure your data has remained private and continues to stay private on the networks we and all other pharmacies us. Alvarado Medical Plaza Pharmacy prides itself in having taken personal data protection as a serious concern and has and always will exceed the standards for data protection of our patrons and clients.
